Beginning in 2025 employers are no longer required to provide Forms 1095-C to their employees unless they are requested. You may request the 1095-C forms by emailing kpauly@homecare-hospice.org, mailing a request to Human Resources, 115 E. Main St., Allegany, NY 14706, or calling 716-372-2106.
